Case Manager - Targeted Case Management (TCM)
Join Wisconsin Community Services, Inc. as a Case Manager in our fast‑paced Community Based Mental Health program. You will work with adults who have serious and persistent mental illness and help them navigate recovery and services.
Job Summary This position is part of our Targeted Case Management program and is responsible for establishing treatment relationships, developing treatment plans, and providing ongoing case management and crisis intervention under supervision. The role requires working primarily in the community and supporting clients in daily living and access to services.
Job Responsibilities
Develop a treatment relationship and formulate treatment plans for clients on your caseload.
Provide psychosocial rehabilitation, symptom monitoring, and supportive counseling.
Monitor client’s psychiatric medication regimen and compliance as directed by the physician.
Offer crisis intervention and supportive counseling to clients in crisis.
Maintain a treatment‑oriented environment and ensure consistency in achieving treatment objectives.
Coordinate utilization of community resources, day programs, and outside health care providers.
Assist clients in financial planning and development of income resources.
Coordinate consumer access to services and all entitlement programs.
Provide treatment‑related education and liaison with client families.
Assist clients in securing adequate housing and train skills necessary to maintain housing.
Minimum Qualifications Bachelor’s degree in a behavioral science field (social work preferred). Master’s degree encouraged. Must complete all WCS background checks, hold a valid driver’s license, have personal automobile and sufficient insurance to meet agency requirements.
Knowledge, Skills and Abilities Good computer skills (Word, Excel, CSP Exchange). Strong organizational skills, ability to multitask, meet deadlines, and work both independently and as part of a team. Some knowledge of mental illness and experience working with individuals diagnosed with mental illness.
Other Job Information Working Conditions Work is performed largely in community settings, including client homes, apartments, and community locations. Hours will be during regular business hours but may vary based on consumer needs, averaging at least 40 hours per week.
Physical Demands The position is mobile with frequent commuting throughout Milwaukee County, client home visits, and office work. Ability to go up and down stairs and lift up to 20 pounds is required.
