Job Description Position: Advanced Practice Provider (FNP or PA-C) Location: Multiple locations as needed Reports to: Medical Director Type: Full‑time
Charlotte Community Health Clinic offers high‑quality medical, dental and behavioral health services for children and adults. We are currently seeking a Physician Assistant or Family Nurse Practitioner to join our team. Responsibilities include providing high‑quality medical care, patient education, strict adherence to HIPAA guidelines, and involvement in Quality Improvement activities.
Responsibilities
Provide medical services to patients, inclusive of all functions necessary to deliver quality medical care.
Develop treatment plans for acute and chronic diseases.
Educate and guide patients on disease prevention, disease management and healthy lifestyle habits.
Provide monitoring and continuity of care between patient visits.
Contribute to teamwork and communications.
Work within a clinical care team to achieve patient and team goals, participate in staff meetings and collaborate with other disciplines.
Collaborate with physician clinical supervisor for chart review and continued education.
Provide administrative, staff development and student supervisory responsibilities as needed.
Participate in Quality Improvement activities.

Qualifications
Master’s degree in Physician Assistant studies or Nurse Practitioner studies.
Successful passage of the Physician Assistant National Certifying Exam or Nurse Practitioner exam.
Current unrestricted PA or NP licensure in North Carolina.
Current DECA licensure.
Preferred: 2–4 years of community‑based primary care experience, especially with low‑income or uninsured patients.
At least 3 years of experience as an APP.
Bilingual in English and Spanish strongly preferred.
Experience with EPIC EMR preferred.
Current BLS certification.
