Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

Provide skilled nursing care in patients homes with a focus on quality and patient-centered services. This contract role based in Indianapolis, IN, involves comprehensive assessments, individualized care planning, and collaboration with healthcare teams to ensure optimal patient outcomes.

Key Responsibilities:

  • Perform thorough OASIS assessments and develop tailored care plans.
  • Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identify changes, and communicate effectively with physicians and care teams.
  • Educate patients and their families on disease management, treatments, and preventive care.
  • Document all care activities accurately using the Home Care Home Base (HCHB) electronic system.
  • Coordinate care with therapists, social workers, and home health aides.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Maintain compliance with regulatory standards for home health care.

Qualifications:

  • Active RN license in good standing for the state of Indiana.
  • Minimum one year of recent home health nursing experience; field experience preferred.
  • Proficiency with Home Care Home Base (HCHB) and experience with OASIS assessments.
  • Strong clinical assessment, critical thinking, and organizational skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently with a flexible field-based schedule.
  • Excellent communication and time management abilities.
  • BSN preferred.
  • Familiarity with Medicare and home health compliance standards is advantageous.

Schedule and Location:

  • Monday through Friday, 8:00 AM to 5:00 PM.
  • Field-based role requiring daily travel to patient homes within an assigned territory.
  • Caseload typically includes 2532 points per week (35 patient visits per day).
  • Mileage reimbursement provided.
