Job Overview The Advanced Practice Provider for the CCU works collaboratively within a multidisciplinary health team and is responsible for providing comprehensive and age‑appropriate direct care and follow‑up to a population of patients.
The components of the role include clinical practice, consultation, professional development, education, and research.
The APP works in conjunction with the Cardiology and CCU intensivist and Heart Failure and Transplant attendings performing patient consults and follow‑up care.
Clinical core privileges will include the management, treatment, ordering, performing, and interpreting labs, radiography, and other diagnostic tests for acute critical‑care disorders in the adult cardiac critical‑care population.
Requirements
MSN or DNP from an accredited nursing program required OR Master degree from an accredited physician assistant program
Active New Jersey RN and APN license OR Physician Assistant license
The ideal candidate has at least three years of acute‑care work experience
Experience in critical care is preferred
APP work experience strongly preferred
Responsibilities
Utilizes advanced assessment skills to manage a cohort of patients
Performs and documents assessments
Collects and analyzes data and formulates a patient‑centered plan of care
Evaluates and monitors the effectiveness of the prescribed treatment plan
Uses the most current evidence to modify the plan of care based on the effectiveness of the treatment and the patient/family response to the plan
Prescribes appropriate pharmacologic and non‑pharmacologic treatment modalities
Serves as an educational resource to the patient/family, nursing staff, and provider team
Acts as a consultant to multidisciplinary health‑care team members
Educates patients/families about self‑management of acute or chronic illnesses, tailoring instructions to patients’ individual circumstances.
Facilitates the patient’s transition within and between health‑care settings, admitting, transferring, and discharging patients when appropriate
Demonstrates a commitment to patient safety by speaking up about safety issues and changing practice to enhance safety
Shares errors or potential errors through system reporting mechanisms
Demonstrates shared accountability for safe practice
Establishes and maintains communication with other health‑care providers and departments to ensure operational efficiency and effectiveness and to promote consistency and continuity of care
Participates in scholarly activities including research and education
Participates in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate educational opportunities.
Participates in the professional development of others
Acts as a preceptor and mentor to support the development of nurses, other nurse practitioners, students, and other health‑care providers
