Location: Walnut Creek, CA
As an Activity Assistant, you will plan, coordinate, and conduct individual, small group, and large group therapeutic activities to enhance resident quality of life. You will assist in fostering effective communication among staff, residents, families, support personnel, government agencies, and the public to meet resident needs.
Responsibilities
Participate in planning and conducting individual, small, and large group activities.
Assist in providing effective communication between employees, residents, families, support personnel, government agencies, and the public.
Participate in community planning for the facility and residents’ needs.
Assist in developing a monthly activity calendar and maintaining attendance records.
Assist the Quality Assurance and Assessment Committee in developing and implementing plans of action to correct quality deficiencies.
Participate in discharge planning and the development and implementation of activity care plans and resident assessments.
Assist in arranging transportation for residents, including outings, appointments, or discharge.
Assist in developing monthly activity schedules, including outings, group activities, and in‑room activities for bed‑bound or isolated residents.
Encourage residents to participate in self‑initiated activities such as hobbies, crafts, and reading.
Provide materials as needed, including Braille or audio books.
Assist with assessment documentation and quarterly progress notes as assigned by the Activity Director.
Keep the Activity Department clean, orderly, and secure.
Qualifications
High school diploma or equivalent.
Preferable one year of experience in a long‑term care facility.
Ability to read technical procedures and policy manuals.
Ability to effectively present information and respond to questions from managers and employees.
Mathematical skills for applying concepts such as fractions, percentages, ratios, and proportions.
Reasoning skills to solve practical problems.
Physical Demands Standing, walking, sitting, reaching with hands and arms, pushing/pulling, talking and hearing, tasting and smelling, lifting up to 50 pounds, climbing, balancing, stooping, kneeling, crouching, or crawling occasionally; and vision: ability to adjust focus.
Work Environment Noise level is usually low to moderate. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
