Hartford Hospital is a Level 1 Trauma Center that provides cutting edge treatment to its patients and hosts one of the largest robotic surgery centers in the Northeast. The Community Care Clinic (CCC) is located at 132 Jefferson St. CCC serves close to 4,000 patient visits annually with an average of 60 patients per day. The service supports inpatient and outpatient infectious disease consults and is Ryan White funded.

Job Summary The Community Care Advanced Practice Nurse or Physician Assistant provides skilled nursing care in a variety of clinical settings, supporting the HHC mission to improve health and healing. Under provider direction, the role prevents transitional care gaps and enhances continuity for patients.

Job Responsibilities

Functions as a member of an interprofessional care team in an expanded nurse practitioner role, helping patients transition from the acute care setting to community care.

Partners with inpatient and ED teams to proactively identify potential transitional care gaps and establish safe transition plans.

Performs post-hospitalization/ED transitional care strategies within 24‑48 h after discharge, including phone calls, education, and medication reconciliation.

Facilitates completion of diagnostic workup and collaborates with homecare, pharmacy, and DME to ensure necessary supplies and resources.

Collaborates with clinic physicians to resolve issues and advance treatment plans until a primary care provider is established.

Documents all communication, transition plans, and patient outcomes in EPIC.

Completes transitional care strategies per CMS/Payer guidelines for Transitional Care Management.

Establishes therapeutic rapport with patients and serves as a patient advocate.

Works independently and collaboratively to provide safe patient care and efficient service.

Attends care team meetings to facilitate safe transition plans or resolve patient issues.

Develops and implements evidence‑based workflows to improve patient experience.

Collects and analyzes program data to identify opportunities and recommend improvements.

Provides office‑based nursing care in collaboration with provider, communicating patient needs and nursing recommendations.

Qualifications

Current APRN or Physician Assistant license required.

Certification in accordance with Connecticut state law.

Specific experience in specialty may be required.

Open to new graduates and experienced APP providers.
