Location: California Start Date: Nov 17, 2025 LOA: 20 days Provider Type Needed: Family Practice Nurse Practitioner Schedule: Monday-Thursday, 8:30 AM - 5:30 PM with a rotating Thursday of 10:00 AM - 7:00 PM

Responsibilities And Duties

Assess and diagnose patients with multiple comorbidities Conduct wellness exams and routine check-ups Deliver direct patient care and minimize unnecessary referrals Collaborate with an integrated care team Provide education on disease management and prevention Utilize EPIC EMR for documentation and patient management

Additional Information

Minimum 5 years of experience in family practice or internal medicine required Must have FQHC or IHS experience Experience with EPIC EMR required Must be licensed as a Family Nurse Practitioner in California Must hold board certification in Internal Medicine and Family Nurse Practitioner (FNP) Candidates should indicate Medicare/Medicaid enrollment status in California
