Job Responsibilities The Ryan White Case Manager is responsible for both medical and non‑medical case management for adults living with HIV, including completing acuity scales, developing individualized service plans, and linking clients to services that address social determinants of health.

Primary Duties & Responsibilities

Complete initial assessment of service needs using the Ryan White Acuity Scale and update per standards.

Develop a comprehensive Individualized Service Plan (ISP) and update per standards.

Monitor client progress toward health goals and efficacy of the care plan.

Continuously assess client needs and personal support systems.

Maintain accurate documentation in eClinical (the medical record).

Document coordination of services, referrals, and follow‑up during service provision.

Ensure timely and coordinated medical and support services.

Provide adherence counseling for complex HIV treatments.

Advocate for client needs and review utilization of services.

Coordinate prenatal care if applicable.

Other duties as assigned.

Minimum Qualifications Bachelor’s degree in Social Work or related field OR Bachelor of Science in Nursing AND one (1) year of case management experience.

Preferred Qualifications

HIV case management experience.

Master’s degree in social work or related field.
