Job Description: As a Peer Support Specialist on the Clinical Case Management Team, you will be an important member of an interdisciplinary team of professionals dedicated to promoting recovery and wellness for individuals living with a variety of mental health and substance use disorders. Using your own lived experiences with recovery, you will build meaningful relationships that empower patients to achieve their personal goals. In this role, you will support patients by offering empathy, encouragement, and practical guidance. You will play an essential in supporting our patients in building recovery capital—the internal strengths and external resources that sustain long-term recovery!

Key Responsibilities: The Clinical Case Management Team Peer Support Specialist provides recovery-oriented support to adults living with Severe and Severe Persistent Mental Illnesses in community and office-based settings.

Responsibilities include:

Working with a multi-disciplinary team including clinicians, psychiatrists, case managers, and more to support patients in achieving treatment goals and objectives

Applying stages of change and motivational interviewing skills to meet individuals wherever they are in their recovery journey

Using lived experience to engage individuals in identifying and working toward goals that build on recovery capital

Providing support to individuals that fosters independence, autonomy, and self-advocacy

Assisting individuals to access sustainable community resources that promote wellness and recovery

Acting as an advocate to reduce stigma and promote resiliency

Attending and actively participate in treatment team meetings and individual and group supervisions

Communicating effectively with peers, managers, and support staff

Maintaining and completing all documentation that is required in a timely manner

Complying with regulatory confidentiality requirements as outlined in HIPAA and 42 CFR Part 2

Providing support that is sensitive to diverse cultures, languages, and recovery models

Possessing a teamwork ethic and behaving in a manner that aligns with high standards of ethical practice

Requirements: Knowledge, Skills, and Abilities:

Must have organizational, written, and verbal skills.

Basic computer experience required.

Successful candidates will be organized self-starters who enjoy working in a fast-paced, dynamic environment.

Qualifications:

Must be certified as a Peer Specialist or Recovery Coach. Must be eligible for CRSW and Intentional Peer Support certificates.

Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy.

Experience: Experience working as a peer specialist in a mental health center setting and/or substance misuse services is highly recommended.

Education: A minimum of a High School Diploma or GED is required.
