Overview
PRN (PCP) Nurse Practitioner (NP) Medical Staffing at Interim HealthCare Inc. We are seeking a skilled and compassionate Nurse Practitioner to join our team in delivering high-quality care to patients. This role involves providing both direct and indirect patient care under the supervision of the Medical Director/Primary Care Physician. The Nurse Practitioner will be responsible for managing complex clinical cases, demonstrating sound clinical judgment and a commitment to patient-centered care. Responsibilities
Performs health histories and physical exams; provides participant teaching. Orders, interprets, and follows up on lab tests and results; documents care in EMR. Initiates emergency measures as indicated and recognizes abnormalities that justify immediate referral. Assesses, initiates, and coordinates participant care on a daily basis under supervision of the attending physician; refers to other physicians including specialists with staff PCP approval. Requests consultations from other services as needed in collaboration with the attending physician and established protocols. Acts as liaison with physicians, nurses and other healthcare professionals, providing education, communication and support for staff, participants, and families. Directs participant management in conjunction with physician staff. Participates in provider call schedule and as a member of the Interdisciplinary Team (IDT); attends staff meetings and participates in participant care planning. Assists the IDT in formulating a plan for the coordination of 24-hour care. Documents participant changes in the medical record and communicates changes to team members. Evaluates and treats participants in their homes, assisted living facilities, nursing homes, and hospitals when medically indicated. Precepts medical students and residents during their rotation as needed. Encourages clients to have Advanced Directives and provides education; discusses Do Not Resuscitate (DNR) options with clients and/or family members as appropriate, and may document DNR orders in the medical record in collaboration with a physician. Qualifications
Master’s degree in clinical nursing from an accredited Nurse Practitioner program. SC RN license and Family Nurse Practitioner certification. 3-5 years nursing experience; Nurse Practitioner experience preferred. Ability to work within the scope of authority and meet standardized competencies for the position. Experience working with frail or elderly populations preferred. Strong clinical assessment, analytical, and problem-solving skills; ability to work effectively in a team environment. Excellent organizational, verbal, and written communication skills. Ability to work a flexible schedule and autonomously while reporting to PCP and Director. Knowledge of physical, mental, and social needs of frail older adults and chronic disease management.
