Stroke Program Navigator We are seeking a dedicated Stroke Program Navigator to join Jackson Memorial Hospital within Jackson Health System. The role is full-time, required to work days.

Summary

The Stroke Program Navigator is responsible for the facility’s stroke education organization and compliance, serving as subject‑matter expert and resource to clinical staff. Under the supervision of the Stroke Program Coordinator and Quality Director, the Navigator conducts concurrent review of stroke patients during hospitalization and after discharge, and educates patients, families, and nursing staff to maintain compliance with stroke center requirements.

Responsibilities

Chair the corporate stroke education committee to ensure all required education is aligned, reviewed, and compliant annually with TJC regulatory guidelines.

Coordinate and prepare planned on‑site stroke service evaluations, designations, and certifications.

Monitor compliance with TJC assessment and reassessment requirements.

Assist stroke team staff in problem solving related to stroke patient care, equipment, procedures, rehabilitation, and discharge plans.

Coach staff to develop, maintain, and enhance clinical skills and decision‑making.

Provide just‑in‑time education when knowledge gaps arise.

Create, evaluate, and deliver formal and informal interdisciplinary education to improve patient outcomes and quality of care.

Develop education for secondary prevention and coping strategies for patients and caregivers through stroke support group and community outreach.

Collaborate with the Stroke Program Coordinator to educate providers from EMS and interhospital settings.

Ensure appropriate education is assigned/provided to physicians, midlevel providers, nurses, etc., and that community outreach education meets hospital regulatory requirements.

Participate in committee meetings requiring stroke representation.

Build strong relationships with patients, families, coworkers, and community to promote a unified workplace.

Perform all duties in accordance with the Comprehensive Stroke Program’s Mission and Vision Statements.

Perform all other related job duties as assigned.

Experience

Generally requires 5 to 7 years of related experience.

Education

Bachelor’s degree in Nursing is required.

Credentials

Valid Florida RN license is required.

SCRN required or obtained within first year of employment.

American Heart Association Basic Life Support (BLS) and any additional applicable life support certification for healthcare providers required upon hire with at least 6 months validity and maintained during employment.

Must meet and maintain valid and current all unit specific and organizational skills/competencies, certifications/licensures, as required by regulatory and/or nursing standard of practice for the specialty.
