Opportunity Information Sutter Medical Group is seeking a dedicated and compassionate Skilled Nursing Facility Advanced Practice Clinician (APC) – either a Nurse Practitioner (NP) or Physician Assistant (PA) – to join our post‑acute care team. This role focuses on delivering high‑quality, patient‑centered care to individuals residing in skilled nursing and rehabilitation facilities. The ideal candidate will have experience managing medically complex post‑acute and long‑term care patients and a strong commitment to interdisciplinary collaboration, continuity of care, and optimizing patient outcomes during transitions of care.
Responsibilities
Provide comprehensive health assessments, ongoing medical management, and coordination of care for patients in skilled nursing and rehabilitation facilities under the supervision or collaboration of a designated physician.
Manage patients with multiple chronic illnesses, acute conditions, and complex medication regimens, following evidence‑based clinical guidelines.
Conduct daily or scheduled rounds in assigned SNFs, ensuring timely evaluation and management of changes in condition.
Collaborate closely with facility staff, nursing teams, rehabilitation therapists, social workers, and case managers to deliver coordinated, patient‑centered care.
Communicate effectively with patients, families, caregivers, and facility staff regarding goals of care, treatment plans, and care transitions.
Ensure compliance with all SNF regulations, documentation standards, and quality measures, including participation in care plan meetings and facility‑based quality improvement initiatives.
Partner with supervising or collaborating physicians, hospitalists, and other specialists to support smooth transitions of care and reduce hospital readmissions.
Participate in quality improvement, population health, and value‑based care initiatives related to post‑acute and long‑term care.
Maintain accurate, timely, and thorough documentation in the electronic medical record (EMR) system in accordance with Sutter Medical Group and regulatory requirements.
Requirements
Current and valid California license as a Nurse Practitioner or Physician Assistant.
National certification as an NP (FNP, AGNP, or ACNP) or PA‑C.
Active and unrestricted DEA registration (or eligibility to obtain).
Demonstrated experience in post‑acute, skilled nursing, long‑term care, or geriatric settings preferred.
Strong clinical skills in managing complex, multi‑morbid patients.
Excellent communication and interpersonal skills with a patient‑ and team‑centered approach.
Ability to work effectively within an interdisciplinary, facility‑based environment.
Proficiency in using electronic medical record (EMR) systems.
Preferred Qualifications
Experience in post‑acute, rehabilitation, or geriatric medicine.
Knowledge of CMS regulations, SNF billing and documentation requirements, and quality metrics.
Participation or familiarity with value‑based care or population health programs.
