Nurse Practitioner / Physician Assistant – Rapid Response Night Per Diem
Hackensack, New Jersey
Shift: Night | Status: Per Diem (Requisition # 2025-170679)
Overview The Advanced Practice Nurse (APN) or Physician Assistant utilizes a patient‑centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision‑making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. Per Diem Night position RRT service; experience in Critical Care or Emergency Medicine preferred.
Responsibilities
Performs and documents accurate, complete, and relevant history and physical assessments appropriate for the patient’s age, gender, and clinical problem.
Orders, performs, and interprets laboratory tests and diagnostic studies/procedures in collaboration with physicians and consultants as appropriate.
Manages general medical and surgical conditions based on comprehensive knowledge of etiologies, risk factors, pathophysiology, and pharmacologic indications and contraindications.
Prescribes medication safely and effectively in accordance with best practices, hospital policy, and NJ regulations, providing patient education as needed.
Promotes Hackensack Meridian Health (HMH) APN practice recognition through conferences, publications, and presentations.
Provides patient‑ and family‑centered care, compassion, and effective treatment of disease.
Adheres to HMH nursing protocols, standards of care, and disease‑specific quality indicators.
Applies evidence‑based and value‑based medicine to systematically analyze and improve patient care practices and outcomes.
Evaluates patient/family transition of care planning, considering resource utilization, cost, quality, safety, and self‑management promotion for chronic conditions.
Contributes to medication reconciliations, participation in annual APN peer review, and the translation of best practice into clinical practice.
Facilitates consistent, coordinated care and clear communication among all members of the healthcare team through daily interdisciplinary rounds.
Advocates for patients and families, identifying and addressing obstacles to care progression and escalating issues as appropriate.
Serves as a clinical resource for the multidisciplinary team by addressing clinical, process, and system improvement opportunities.
Collaborates with medical staff to support nursing‑led protocols and mentors other APNs and students in APN programs.
Promotes a collaborative work environment supporting the organizational mission, values, and pillars.
Engages in professional development, clinical research, and dissemination of findings through practice, education, presentations, and publications.
Ensures compliance with all applicable statutes, professional standards, and HMH policies.
Qualifications Required:
Graduate of an NLN/AACN accredited nursing program.
Master’s or Doctorate degree from an accredited Nurse Practitioner program.
Maintains clinical competencies through continuing education and clinical practice.
Experience in a relevant patient‑care setting (or as regulations dictate).
National Provider Identifier (NPI).
Prescriptive authority through CDS and DEA registration according to New Jersey state law.
Collaborative agreement with physicians on staff.
Excellent written and verbal communication skills.
Proficient computer skills, including Microsoft Office and/or Google Suite platforms.
NJ State Professional Registered Nurse License.
Advanced Nurse Practitioner (w/ Master’s Degree).
BLS AHA Basic Health Care Life Support HCP Certification.
Board certification by AANP or ANCC.
Preferred:
Advanced Cardiac Life Support Certification.
Physician Assistant:
Successful completion of an approved physician’s assistant program accredited by the AMA Council on Medical Education.
Physician Assistant License, National Commission on Certification of Physician Assistants.
National provider identifier (NPI).
BLS AHA Basic Health Care Life Support HCP Certification.
Board certification as required for state licensure.
Compensation Minimum rate of $79.05 hourly. HMH is committed to pay equity and transparency. The posted rate of pay is a reasonable good faith estimate of the minimum base pay for this role at the time of posting.
