Job Description We are seeking a dedicated and detail-oriented Pharmacist to join a Long Term Care Pharmacy team. This role focuses on data review and order fulfillment. The ideal candidate will bring a strong foundation in pharmacy practice and a passion for delivering high-quality care to long-term care patients.

Responsibilities

Perform clinical data review to ensure accuracy, safety, and compliance with regulatory standards.

Collaborate with pharmacy technicians and healthcare providers to ensure optimal patient outcomes.

Maintain accurate documentation and participate in quality assurance initiatives.

Pharmacy

PharmD

Compounding

Prescription review

Order Verification

Prior authorization

Additional Skills & Qualifications

PharmD from an accredited college of pharmacy.

Active Minnesota Pharmacist License in good standing.

Minimum of 1 year of pharmacy experience, preferably in long-term care.

Pay and Benefits The pay range for this position is $62.00 - $65.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment. Benefits are subject to change and may be subject to specific elections, plan, or program terms.
