The salary range for this position is $37.76 - $52.87 hourly. Placement within the range depends on experience and internal equity. Description The

Ambulatory Social Worker

reflects the mission, vision, and values of NM and complies with all relevant policies and regulations. The Ambulatory Social Worker II collaborates and participates in the coordination of screening, assessment, and care‑planning activities with patients and families who have complex psychosocial needs, while supporting continuity of care as needed. The role utilizes expertise and problem‑solving skills to achieve optimal clinical outcomes within effective resources and timeframes. The Social Worker provides psychosocial assessment, intervention services and ongoing support to patients who meet program criteria.

Responsibilities

Partner with ambulatory nursing and pharmacy teams to achieve system goals and objectives around care coordination for NM populations with complex clinical and psychosocial needs.

Develop and implement appropriate assessment‑based treatment plans.

Lead the process on Advanced Directive completion.

Inform and coordinate with interdisciplinary clinical teams to ensure high quality of care and joint decisions in the patient’s best interest.

Apply specialized clinical knowledge and advanced clinical skills in the areas of assessment, diagnosis, and treatment of mental, behavioral, addictions disorders or conditions and make appropriate referrals or transfers to the correct level of care.

Communicate in a calm, active‑listening manner.

Utilize translators appropriately to facilitate understanding.

Identify solutions or changes in the patient plan of care or community resources.

Use critical thinking to identify and screen high‑risk social cases.

Provide treatment planning, psychosocial interventions, health education, financial counseling, referrals, and discharge/transition planning.

Provide resources, including educational materials, to patients and families to support the care plan.

Analyze patient situations for appropriate care provision.

Address social and emotional needs that may impact medical treatment response.

Provide timely and effective interventions in emergency and crisis situations.

Serve as a lead resource and assist with referrals for charity and financial resources, guardianship issues, family problems, or competency issues.

Maintain information and referral lists for post‑acute care resources.

Use effective service‑recovery skills to solve problems.

Educate team members on socio‑economic, emotional, and cultural issues that impact care delivery.

Advocate for patient and family.

Telephonically and in person meet with patients, families and care team members to coordinate psychosocial, financial, housing and transportation needs.

Collaborate with team and community linking for patients with complex social, economic and emotional needs.

Participate in care conferences and rounds.

Keep team apprised of current state and plan of care.

Collaborate with care team to identify at‑risk populations and improve care delivery.

Partner with post‑acute care providers to create seamless discharge plans.

Know insurance and reimbursement processes.

Demonstrate teamwork across departments.

Communicate effectively, respecting diverse opinions and acknowledging contributions.

Qualifications Required

Master’s degree in Social Work (MSW)

Licensed Social Worker (LSW)

Three to five years of experience in social work.

Preferred

Licensed Clinical Social Worker

External candidates and candidates who have not worked at NM within the last year receive a $4,000 sign‑on bonus. Internal candidates and rehires within the last year are ineligible.
