Nestled in the heart of the vibrant East Bay area of California, we are on a mission to revolutionize the way home health is perceived and delivered.**Muir Home Health is growing and looking for an experienced and compassionate Speech Language Pathologist (SLP) to join our team!** As an SLP with Muir Home Health, you will have the opportunity to advance your career while receiving strong compensation and excellent benefits.**BENEFITS*** Competitive compensation* Health, dental, vision, life, and disability insurance* 401(k) plan with generous company match* Critical illness benefit* Paid time off* Employee assistance program**Pay Range:** $65.00 - $85.00/hr**JOB SUMMARY**The Speech–Language Pathologist is responsible for implementation of standards of care for speech language pathology services.**DUTIES AND RESPSONSIBILITY*** Conducts appropriate evaluations, assesses home environment (as appropriate), and identifies equipment needs relative to speech/language function.* Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative, and caregivers.* Provides services that are ordered by the physician as indicated in the plan of care.* Provides patient, caregiver, and family counseling.* Provides patient and caregiver education.* Prepares clinical notes.* Communicates with the physician who is responsible for the home health plan of care and other health care practitioners related to the current home health plan of care.* Identifies functional speech defects and establishes a plan of care to improve patient’s function.* Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.* Recommends communication devices/aids.* Participates in case conferences, in-services and other meetings required ensuring coordinated and comprehensive care.* Maintains clinical competency in speech language pathology practice and theory.* Identifies patient and family/caregiver needs for other services* May supervise home health aide in procedures directly related to the speech–language therapy plan of care.**JOB REQUIREMENTS*** Valid CA Speech Pathologist License* 2+ year of clinical experience as a speech therapist* 1+ year of working with the adult population in speech therapy* Maintain CPR Certification* Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile.Muir Home Health **is part of the Cornerstone Group**with about 75 home health and hospice agencies throughout the country. **While we are part of a large family, we operate as a local team.**We understand we are nothing without great employees!
