Duties/ Responsibilities
Assist Clinical team in providing follow up services to clients placed into housing to ensure successful transition.
Provide after-care services to residents with the focus on income and housing retention.
Collect and make available data regarding client retention for the purpose of reporting, assessment and evaluation.
Develop and facilitate housing, entitlement retention workshop and strategies.
Work with current residents with their housing search and continue working with all residents as they transition into the community.
Collaborate with other service providers and housing providers to assist discharged residents to continue to reach their housing and income goals.
Perform initial intakes when needed.
Related duties as assigned.
Hours Full-time 37.5 hours per week
Monday-Friday 10am-6:30pm
Qualifications BA preferred but can be substituted with some work experience. High School diploma or GED required. Should possess strong background in the fields of homelessness, addiction, mental illness and/or entitlements. Previous experience preferred in the areas of housing and employment retention. Excellent writing and oral communication skills and computer proficiency required. Must be able to work in a high-stress, high-volume environment, while maintaining positive open communications with other components of the treatment team.
Vaccination preferred but not required
