Responsibilities
Facilitate group and individual therapy sessions for patients struggling with addiction.
Conduct intakes and assessments to gather patient information and guide treatment plans.
Collaborate with the treatment team to develop and implement individualized care plans.
Maintain accurate and compliant patient records in accordance with 245G and 1115 Demonstration Waiver requirements.
Participate in clinical consultations and staff meetings to ensure continuity of care.
Requirements
Current and active Licensed Alcohol and Drug Counselor (LADC) credential in Minnesota.
Experience providing substance use disorder counseling and case management
Strong documentation skills and experience using electronic health record (EHR) systems.
Ability to respond to on‑call requests within a specified timeframe.
Excellent communication, crisis intervention, and problem‑solving skills.
Compensation $70–75K/Year with a $7,500 Sign on bonus
Actual compensation may be determined by various factors such as licensure, experience, skillset, internal equity, schedule and/or location. Estimates are factored on a full time FTE.
