ASVH-Flagstaff – LPN/RN Wound Care Nurse
Location:
2100 N Gemini Rd, Flagstaff, AZ
Schedule:
Flexible (7am–3pm, 8am–4pm, or 8am–5pm), Monday–Friday, 8 hours/day. No weekends.
Position:
1 Full-Time
We are seeking a
Licensed Practical Nurse (LPN) or Registered Nurse (RN)
to provide specialized wound care and support residents’ skin health.
Key Responsibilities
Perform wound assessments, management, and reassessments per policies and procedures.
Develop and implement skin care programs to prevent breakdown.
Manage residents with draining wounds, fistulae, ostomies, and incontinence.
Revise and evaluate plans of care; assess and manage pain.
Serve as a resource for nursing staff and provide in‑services on wound care, ostomy care, and prevention strategies.
Communicate effectively with staff, physicians, and interdisciplinary teams.
Maintain accurate documentation and stay current on wound care advancements through training and continuing education.
Requirements
Current Arizona RN or LPN license.
Certification as a Wound, Ostomy, and Continence Nurse (CWOCN) or Certified Wound Care Nurse (CWCN).
CPR/BLS certification.
Fingerprint Clearance Card issued by the Arizona DPS.
In‑state travel as needed.
Preferred
Experience in a skilled nursing or long‑term care facility.
Required Documentation for Hire Resume, Fingerprint Clearance, Background Check (7 years, OIG/SAM), CPR/BLS Card, TB Test, License/Certification, Skills Assessment, Hepatitis B vaccine or declination, Competency Tests, Drug Test, COVID Vaccination, APS.
