For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.

The CRED workforce initiative provides a vital support to young adults ages 18 to 40. The program aims to serve as a pathway to help unemployed, underemployed, or individuals impacted by community violence to gain a long‑term employment and economic stability. Through CRED, participants engage in work readiness training, sector-based occupational training with a focus on construction and healthcare career pathways. The programme integrates wraparound supportive services, including case management, mental health counselling, academic support, and job placement assistance, to ensure participants can fully engage and remain successful throughout their training and transition into full‑time employment.

STATEMENT OF THE JOB Under the direction of the Program Manager, the CRED Case Manager I renders professional intervention to clients in need of assistance through comprehensive support services (engagements, assessment, intake, application assistance, connection to services and resources to meet their goals, retention and follow‑up).

Provide case management services, assessment and intake of participants to determine eligibility for services, including responding to telephone and walk‑in inquiries, conduct pre‑screenings and provide referrals for those not eligible.

Conduct initial assessment to identify participant’s strength, need and goals in collaboration with the mental health counsellor, job developer and outreach/retention specialist.

Develop individualised case plans with participants, outlining goals, objectives and strategies for success.

Provide ongoing support and guidance to participants, addressing barriers and challenges.

Facilitate connections to community resources, services, and support networks. Refer participants to relevant resources, services, and opportunities.

Monitor participant progress, adjusting case plans as needed. Conduct field visits to training sites, participants internships and/or employment.

Offer weekly one‑on‑one coaching and counselling to participants, focusing on goal setting, problem‑solving, and decision‑making skills in the field and office.

QUALIFICATIONS

Bachelor’s degree in social work, Human Services, Counseling, or a related field or AA degree in social work, Human Services, Counseling, or a related field with two years’ related field experience in City systems such as justice, public assistance, child welfare, housing or other relevant social service setting providing direct services.

Proven experience in case management, counselling, or a related field, working with the target population or vulnerable populations.

Must be available to work in person 5 days a week.

Ability to work flexible hours, including weekends, evenings according to the needs of program operation.

Able to conduct home/field visits and travel throughout the five boroughs as needed.

Ability to utilise public transportation, climb stairs, walk.

Frequent sitting, standing, & walking.
