Responsibilities
Assessing patients’ condition and implement nursing care plan.
Performing nursing procedures in patient's home such as wound dressing and management, removal of stitches, venipuncture, changing of urinary catheter and etc.
Ensuring continuity of treatment plans for home care patients.
Maintain accurate documentation of procedures carried out and medications dispensed.
Any other ad-hoc duties within the team when necessary
Requirements
Candidate must possess at least a Diploma in Nursing and be registered with Singapore Nursing Board.
