Base Pay Range
$125,000.00/yr - $158,593.75/yr
Principal Duties and Responsibilities
Performs independent outpatient office visits and manages a panel of heart failure patients, including heart failure hospital discharges, acute and chronic heart failure patients, LVAD patients, heart transplant recipients, patients listed for heart transplant, and patients on chronic inotropic support.
Demonstrates the ability to understand heart failure disease processes, interprets laboratory results and diagnostic studies (including but not limited to echocardiograms, cardiac catheterizations, cardiac MRIs, and stress testing).
Has the ability to assess and apply evidence‑based medicine in conjunction with ACC/AHA heart failure practice guidelines. Provides patient and family education regarding heart failure disease process and medications.
Participates in advanced heart failure consultation service at Rhode Island Hospital, carries heart failure service pager, and responds to inpatient heart failure consults. Makes assessments and develops plans of care for heart failure patients as part of a shared visit with a heart failure cardiologist.
Coordinates and collaborates with other members of the heart failure team as well as hospital care team. Participates in weekly heart failure team meetings.
Completes yearly mandatory continuing education requirements including BLS and ACLS.
Participates in the education, clinical research and mentoring of healthcare providers.
Performs other duties as assigned.
