Job Overview Responsible for reviewing discharged patient records and indexing charts to correct encounter and document types, while analyzing both paper and electronic formats to determine incomplete or missing items. Tasked with identifying healthcare practitioners accountable for addressing any deficiencies. Collaborates with various hospital departments and physician office staff to procure any necessary corrected or missing documents. Assists in calculating and charging patient observation charges, as well as reviewing and correcting billing error reports. Completes quarterly submissions to outside reporting entities. Additionally, assists with front office responsibilities as needed, including medical record releases, answering phones, greeting in-person customers.
Job Requirements
Required:
High school diploma or equivalent.
1 year of experience in acute care setting with primary responsibility for maintaining documentation requirements.
3 months of on-the-job training required to become proficient with hospital record-keeping systems.
Proficient in oral and written communication, customer service, organization, decision making, excellent time management, and interpersonal relations.
Proficient in Microsoft Office programs, including Outlook, Word, PowerPoint, and Excel.
Preferred:
Registered Health Information Technician (RHIT) through the American Health Information Management Association (AHIMA).

Verifies receipt of discharged patient charts as listed on the daily discharge and census reports and identifies as well as retrieves any unaccounted charts.
Reviews and analyzes patient charts for accuracy and deficiencies, including missing documents, signatures, or text per regulations mandated by medical staff, government agencies, and accrediting organizations.
Prepares, scans, and indexes paper charts to correct encounter and document type.
Ensures that the patient type registered in the electronic medical record (EMR) aligns with the documentation in the patient record. Communicates with the relevant department to address any necessary changes or corrections.
Assigns and monitors queries, oversees reanalysis, and reassigns deficiencies as needed. Regularly communicate with physicians regarding queries and deficiencies in accordance with the Medical Staff Rules and Regulations. Assists physicians and clinicians with completion of deficiencies as needed.
Ensures the completion of dictated physician reports and monitors daily for any required corrections. Assists with transcription system education and addresses technical issues as they arise.
Assists in calculating and charging patient observation charges.
Assists in reviewing and correcting patient billing errors.
