Social Worker/MSW Per Diem (Washington Township)

Jefferson Health . This is an acute care medical–surgical discharge planning position (not a psych/Behavioral Health or counseling role). Provides social work services and collaborates with the multidisciplinary health care team to establish a discharge plan for the patient. Responsibilities

Interact with co-workers, visitors, and other staff in a manner consistent with the values of Jefferson. Conduct psychosocial assessment and reassessment of patients and family/significant others related to social, emotional, and cognitive needs in relation to illness and health maintenance. Develop and implement a plan of intervention addressing identified needs from assessment. Collaborate with the multidisciplinary team to ensure appropriate discharge planning. Provide counseling for patients, families, and significant others to assist with coping with new diagnoses, chronic illness, and palliative or terminal care. Advocate on behalf of patients/families to obtain necessary services. Support acute care discharge planning activities. Requirements/Availability

Must be willing to float to all 3 NJ locations. Weekday availability to orient; after orientation, commitment is either 3 weekday shifts and 1 weekend shift every 4 weeks OR 2 weekend day shifts every 4 weeks. Major holiday commitment. Salary and Benefits

Salary Range : $30.41 to $43.33 per hour. Actual hiring rate based on experience, skills, and qualifications. This position is not eligible for an annual incentive.
