Location: Tuscany Plaza Health Clinic, Yuma, Arizona
Job Overview Summary : The Social Worker supports the physician and interdisciplinary team in the provision of patient care, enhancing quality of clinical outcomes and patient satisfaction while managing cost of care and providing timely, accurate information to payers. This role integrates and coordinates utilization management, care facilitation, and discharge planning functions.

Pay Rate Type : Hourly
Salary Range : Min = $27.89 | Mid = $34.86 | Max = $41.83 per hour
Location : Tuscany Plaza Health Clinic, Yuma, Arizona
Responsibilities
Conduct screening and assessments of patients’ psychosocial risk factors, evaluating prior functioning levels and support systems; assist in identifying and managing victims of abuse, neglect, domestic violence, rape, etc., and those requiring crisis intervention.
Intervene in cases involving child abuse/neglect, domestic violence, elderly abuse, institutional abuse, and sexual assault; follow hospital policy on child abuse/neglect, domestic violence, guardianship, foster care, adoption, mental health placement, adult/elderly abuse, child protection, and sexual assault.
Communicate with patients and families regarding emotional, social, and financial impact of illness and/or disability; assess and mobilize resources; conduct family meetings to establish social service goals, provide counseling, assist in problem solving, and make community referrals.
Educate patient/family and physician regarding post‑acute options and adaptation to diagnosis and treatment.
Participate in discharge planning for complex patients, ensuring timely discharge and appropriate linkage with post‑discharge care providers; document assessments, plans, interactions, and interventions per guidelines.
Assist Registered Nurse Case Managers with discharge planning; coordinate when significant or intensive community resources are necessary; review, evaluate, and prioritize referrals.
Serve as liaison with community agencies; lead in obtaining charity, financial resources, legal guardianship, adoptions, psychiatric referrals, competency, palliative care, and other needs.
Perform other duties as assigned.
Minimum Education Required
Master’s degree in Social Work
Preferred Experience
