Social Worker - Medical/Dialysis (MSW)

Scope of Position / Position Summary Social work services include psychosocial assessments, mental health counseling, crisis intervention, financial counseling, discharge planning, health education, provision of material resources, and linkage with community agencies. Services are provided to patients/families to meet their medically related social and emotional needs as they impact medical condition, treatment, recovery, and safe transition from one care environment to another. Core competencies include psychosocial assessment, counseling and crisis intervention skills, knowledge of post‑acute levels of care and the PASAAR process for extended care facility placement, assessment and counseling processes involved, and skills assessing neglect/abuse, including substance abuse and domestic violence.
Confer with the renal patient, the patient’s physician, discharge planners and social workers, and appropriate hospital staff regarding post‑discharge plans, including, but not limited to, the start of out‑patient dialysis, transition to outpatient dialysis setting, referral to nephrologists in the community, and information regarding vascular access services.
Minimum Qualifications For hire: Master of Social Work from an accredited Graduate School of Social Work; State of Ohio Social Work licensure (LSW).
