Responsible for clinical social work services to the culturally diverse and all ages population supporting the hospital's mission and philosophy. Performs clinical assessment and reassessment of patients and their family needs. Provides crisis intervention and brief counseling with special attention to the psychosocial and psychiatric effect of illness and disability, including issues of death and dying. Participates in multidisciplinary patient care planning and ensures continuity of care by identifying long term needs and coordinating referrals to appropriate hospital and community resources. Provides consultation, education, and psychosocial support to other health care professionals. Qualifications
Must have experience in a health care or related setting and have knowledge of illnesses in relation to social work functions. Must possess adequate understanding of the community health, welfare, and social service agency network. Must have adequate knowledge of managed care. Must understand the purpose and function of social work in health care and the role of social worker as a clinical consultant, discharge planner, counselor, and teacher. Minimum Requirements: Master of Social Work degree Current State of Arizona Licensed Clinical Social Worker (LCSW) OR Licensed Master Social Worker (LMSW) by the Arizona Board of Behavioral Health Examiners Preferred: Previous experience with Meditech.
