Undertake comprehensive social care assessments and produce detailed support plans as needed. Utilize your knowledge and understanding of Direct Payments using a strength-based approach. Apply sound knowledge of the continuing health care process and legislation, including Decision Support Tools. Engage sensitively and effectively with family members of service users. Be involved in CHC Assessments, Asylum/NRPF cases, Transition, and Safeguarding tasks.
