Dialysis Social Worker

As a Social Worker at U.S. Renal Care, you will be an integral part of a cross‑functional team helping patients with kidney disease achieve maximum social functioning and psychological adjustment to dialysis treatment and rehabilitation.
What you will be doing Advocate & Support.
You will conduct patient assessment and care planning activities, assess psychosocial needs, complete the KDQOL, counsel patients and families, coordinate resources, and represent patients with local, state, and federal agencies.
Teamwork.
As part of the interdisciplinary clinic team, you will promote teamwork, educate staff, provide training on psychosocial care, and participate in continuing education and staff meetings.
Safety & Quality.
You will help improve clinical and operational processes, participate in QAPI activities, and ensure compliance with all applicable laws and regulations.
What we're looking for
Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).
Current licensure in good standing in the applicable state (unless employed in AZ, PA, or Guam).
Demonstrated working knowledge of English and strong verbal and written communication skills.
Basic computer skills, including Microsoft Office.
Proficiency in all U.S. Renal Care clinical applications within 90 days of hire.
Preferred
Previous experience providing social services to dialysis patients.
Other Requirements
Must meet any practice requirement(s) for the applicable state.
Additional license requirements may apply depending on the state.
