Social Worker MSW

Medical City Healthcare . We are seeking a Social Worker MSW to provide non-clinical, biopsychosocial assessments and interventions on inpatient floors and the outpatient emergency department. The MSW performs crisis intervention, patient/family interventions, high-risk screening, focused emotional support, and referrals for financial assistance or other identified resource needs. The MSW assists in post-acute care placement on all discharges, with emphasis on patients with post-discharge behavioral health and patient assistance/resource needs. The role supports patient and family-centered care and engages in creative, compassionate problem-solving. The scope includes units such as intensive care, Trauma, Oncology, NICU, Transplant, and ED. The MSW responds to physician, non-discharge planning social work consults throughout the facility.
Responsibilities
Work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff to proactively identify and address patient needs.
Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
Maintain knowledge of Medicare, Medicaid, and private payer regulations and processes; understand managed-care concepts and length-of-stay management related to patient and organizational fiscal responsibility.
Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
Coordinate and facilitate meetings between the care team and patients/families/caregivers; assist patients and families in difficult decisions while honoring preferences and values.
Intervene for psychosocial needs, provide support for coping with chronic illness, and engage in consultation, discharge planning, education, counseling, advocacy, and information referrals.
Provide emotional support to patients and family members during complex or emotionally charged issues (end-of-life, death notifications, crisis interventions).
Qualifications
Master’s Degree in Social Work required
LCSW (Clinical Social Worker) or LMSW/RCSW-I (Licensed Master Social Worker/Registered Clinical Social Worker-Intern) in the State of TX
Certification in Social Work preferred
Minimum of 1+ years medical social work experience
Job Details This is a Full Time Day Shift position, 8:00am - 4:30pm, Monday - Friday with some weekends as needed.
