Employment Specialist ACT (22304)

Overview
As an employment specialist for the ACT program, you will support the employment needs for ACT enrolled participants in Warren, Washington and Saratoga counties. You will help participants identify interests and skills related to employment. You will provide services including preparation for employment, job training, developing community relationships with potential employers or vocational agencies, and educating community employers about serious mental illness. You will report to the Director of ACT and will work onsite with community travel.
Responsibilities
Engage individuals and establish trusting, collaborative relationships directed toward the goal of employment in community job settings.
Provide job development and job searching services directed toward positions individualized to the interests and uniqueness of the people on their caseload, following the procedures of the individual placement and support approach to supported employment.
Prioritize vocational skills and independent living skill development during routine client sessions.
Provide individualized support to assist participants in securing and maintaining employment.
Support client needs surrounding insurance, identification, additional income, and other related needs to encourage independent living and personal success.
Arrange access to benefits counseling and additional supports needed.
Share a caseload of approximately 48 adults (residing in Saratoga, Warren and Washington Counties) with the ACT team members; responsible for the oversight and completion of their treatment planning, clinical documentation, charting, and all necessary OMH, ACT, and Agency protocols.
Participate in daily meetings surrounding case reviews, crisis intervention, clinical formulation, treatment planning and additional client/program needs.
Participate in the agency trainings, that in turn support our client's needs, as assigned by Director.
Must be willing to transport clients, in the agency vehicles, when applicable/appropriate.
Meet with employers to provide psychoeducation about serious mental illness and develop plan on how to best support client's and employer's needs.
Other tasks as assigned.
Requirements And Education
Bachelor's degree in mental health or a related field and at least 1+ year related experience required.
A valid NYS driver's license required.
