Overview

The Medical Social Worker is responsible for working in a team structure with the RN Case Manager and other Integrated Care department staff to effect overall care coordination for the transition of care and complex discharge planning process. Through psychosocial assessment and intervention strategies, the role assists patients and their families in understanding and learning to cope with illness and disability. It also assists families in resolving complex personal and environmental difficulties which interfere with effective social functioning and obtaining maximum benefit from medical care. Participates as a professional team member in patient care planning along with hospital and community agency personnel. Provides education and consultation to health care professionals and the community regarding the impact of psychosocial factors. Responsible for the delivery of social work services to the patient, the patient’s family, and other persons significant to the patient; services are well organized, directed, and integrated with other units and departments of the hospital and community. This position collaborates interdepartmentally with all divisions of the hospital (inpatient, ambulatory, and Emergency). Social Workers may assume additional leadership duties to help with onboarding, training, coaching, and job shadowing for newer staff; participate in the department’s peer mentoring program for new staff around developing social work skills; and complete additional special projects as requested. LISW social workers are eligible to volunteer work time to provide group supervision for independent licensure preparation for dependent licensed social workers and students, as well as attend other leadership opportunities as able.
