Within a team environment, provides person-centered consultation, advocacy, resource information and service coordination for people with developmental disabilities who require more involved and specialized assistance. Responsibilities
Service Coordinator I (Entry Level) – Those With Less Than One Year Of Experience Will Not Be Expected To Provide The Specialized Assistance Needed For Those Listed Below Assist with individuals in active transition due to graduating from school in one to two years, individuals in residential placement or living independently, newly eligible individuals, transfers from other Regional Centers, reactivated cases, individuals with dual diagnosis, and cases requiring clinical or forensic expertise Self-determination and Medicaid Waiver cases, Social Security/Medi-Cal problems that require advocacy and appeal process Support completing required HCBS Waiver certifications and documentation Qualifications
Knowledge of developmental disabilities Awareness of entitlement and benefit programs and of public benefit programs Problem-solving ability Ability to make verbal and written presentations to groups Organizational and time management skills Ability to work independently and within a team Responsive to supervision Able to understand and adhere to applicable laws, regulations, policies and procedures Ability to read/speak/write Spanish and English Minimum Qualifications
Master’s or Bachelor’s degree in human services field or a related area, with 2 years of applicable experience in mental health or social work 2 years experience in case management within a related field 1 year experience with individuals with developmental disabilities preferred Valid California driver’s license and reliable, insured vehicle or equivalent means of transportation Able to travel to any location at any time Able to read/speak/write Spanish and English Secondary Job Functions
Conduct strength-based assessments that communicate an understanding of the unique issues facing family members Involve individuals we serve and family in all aspects of person-centered planning and support Design, document, implement and monitor services and supports in a timely manner Facilitate planning team progress, identifying outcomes, needed services and supports Develop and maintain a network of vendors and community agencies Provide relevant information and training about developmental disabilities, services and resources Use effective communication and interpersonal skills that consistently respect children and adults with developmental disabilities and their families Demonstrate knowledge of family/natural support and private/public resources and integrate these into service planning Demonstrate knowledge of entitlement and benefit programs Demonstrate knowledge of the Tri-Counties Regional Center system, the legal system, individual civil rights and overall advocacy Work cooperatively as a team member and develop relationships with community representatives Participate in professional growth through in-service and other training activities Conduct all activities in a professional and ethical manner Additional or different functions may be assigned from time to time Knowledge, Skills, And Abilities

Position requires the ability to work independently in a standard office setting, with mobility to use standard office equipment. This is primarily a sedentary role; may involve grasping, repetitive hand movement, finger dexterity and data entry. Vision and hearing requirements as described, with the ability to communicate in person, before groups and over the phone. May require lifting up to 10 lbs frequently and up to 20 lbs occasionally. May involve standing, walking, reaching, twisting, bending and other movements associated with daily office activities. Work is in an office setting with moderate noise levels and no direct exposure to hazardous substances. Benefits
