Scope/Position Summary
The Social Work Lead works under the direction of the Care Management Leadership Team leading team in providing services directed to achievement of positive patient outcomes, maximizing the patient’s involvement in his/her own plan of care, improving service levels, reducing length of stay and improvement cost management. In addition to basic standards of Acute Case Management, the Lead is expected to role model and exceed staff Standards of Performance. The Social Work Lead assists department leadership with planning, organization, implementation, and evaluation of care management services. Social Work services provided include psychosocial assessment, mental health counseling, supportive care screening, crisis intervention, financial counseling, discharge planning, health education and the provision of material resources, and linkage with community agencies. Social Work Lead provide clinical expertise support to staff, serves as a resource , and mentors staff in the practice of social work and provides direct social work services, as needed. The Social Work Team Lead will be able to demonstrate knowledge of growth and development to coordinate patient care appropriate to the age and health equity of the patient. Minimum Qualifications
Master of Social Work from an accredited Graduate School of Social Work, State of Ohio Social Work licensure. LISW required At least 3-5 years of experience. Location: Rhodes Hall (0354) Position Type: Regular Scheduled Hours: 40 Shift: First Shift Final candidates are subject to successful completion of a background check.
