Overview
Palliative Care Social Worker – Location: Lexington, KY. Schedule: Monday–Friday 8am–5pm. Responsibilities
Provide direct casework and counseling services to the patient and family unit within the community-based palliative care (CBPC) team. Assess psychosocial problems, define treatment goals, and work to alleviate identified issues for both adult and pediatric palliative care teams. Collaborate as part of the interdisciplinary team and contribute to patient care planning under the direction of team leaders, program leaders, and the Chief Medical Officer. Plan and coordinate patient care activities with the palliative care team. Qualifications
Graduate of a Bachelor’s or Master’s program in Social Work accredited by the Council on Social Work Education; licensed in Kentucky as an LSW, CSW, or LCSW. Minimum of two years’ social service experience in a health care setting or social service agency; experience on an interdisciplinary team preferred. Knowledge of death, dying, and bereavement; basic understanding of psychosocial and legal considerations for terminally ill patients and families; and interpersonal dynamics within an interprofessional healthcare team. Ability to exercise judgment and discretion in confidential matters. Ability to make home visits across varied settings; must be able to drive for extended periods and maintain a valid driver’s license with appropriate auto insurance. Strong written and oral communication skills; ability to give and receive constructive feedback.
