Job Responsibilities

Respond to referrals from the interdisciplinary team and engage patients and families in a helping process.

Complete psychosocial assessments and develop individual plans of care that promote continuity of care.

Provide counseling, advocacy, and education to patients, families, and the healthcare team regarding emotional, psychological, social, financial, and developmental aspects of care.

Facilitate discharge planning and transitions to the next level of care, including referrals to community resources.

Collaborate with medical staff and other professionals on psychosocial issues and contribute to quality and safety initiatives.

Maintain timely, clear documentation and support the ongoing development of the Social Work Department.

Qualifications

Master of Social Work (MSW) from an accredited School of Social Work.

Licensed by the State of Ohio Social Work Board (LSW, LISW, or LISW‑S).

1–2 years of equivalent experience (preferred 3–5 years).

For employees in UCMC Psychiatric Services: NCI‑APS Certification obtained within 90 days of hire.

Strong interpersonal skills, crisis management experience, and a commitment to patient advocacy.
