Clinical Manager of Care Management - LISW

Join AIDS Healthcare Foundation (AHF) as the Clinical Manager of Care Management - LISW. The role oversees Care Management Services at a managerial level for your assigned EMA/TGA, applying a strong understanding of Ryan White Services, contract deliverables, and the Standards of Care to advance program and patient goals. You will collaborate with the AHF Healthcare Team, including Providers, the Practice Manager, the Regional Director, and your contract manager to ensure operational processes, provide guidance, resources, and access to care for all patients. You will monitor the Care Management Information System and help keep programs audit-ready while supporting your team’s performance and patient outcomes. Responsibilities

Oversee Care Management Services at a managerial level in the assigned EMA/TGA, ensuring alignment with contract deliverables, Standards of Care, and program goals. Provide guidance and operational support to the Care Management team; monitor activities in the Care Management Information System; ensure audit readiness. Conduct 1:1 meetings with team members to review performance, address challenges, and collaboratively resolve issues. Collaborate with the Healthcare Team (Providers, Practice Manager, Regional Director, and contract manager) to deliver resources and access to care for all patients. Prepare summaries of program operations for the Regional Director of Care Management and contribute to HAB measures and program improvement efforts. Remain aligned with the Ryan White Care Act and safety-net objectives for uninsured HIV/AIDS patients; contribute to contract deliverables and regulatory requirements. Lead the team in implementing the Model of Care and maintaining an evidence-based, compliant program. Qualifications

Licensed Independent Social Worker (LISW-S) in the State of Ohio. Minimum of 3 years of Case Management experience; 2+ years of management experience preferred. Ability to travel up to 30% of the time within the region. Strong understanding of Ryan White Services, contract deliverables, and regulatory requirements. Benefits
