Schedule PRN (as needed) Monday-Friday 8:00am to 5:00pm
About The Role An employee in this position provides direct casework and counseling services to the patient/family unit wherever they may be within the hospice program of care. The Social Worker will provide an assessment of psychosocial problems; define treatment goals and work to alleviate such problems. The Social Worker serves as part of the interdisciplinary team and shares responsibility for other aspects of patient care except where limited by licensure. Plans and coordinates patient care with the primary nurse under the direction of the Counseling Supervisor, Executive Director, and/or Regional Counseling Supervisor.

Requirements
Graduate of a Bachelor’s or Master’s program in Social Work accredited by the Council of Social Work Education; licensed by Kentucky as an LSW, CSW, or LCSW.
Minimum two years social service experience in a health care setting or social service agency; interdisciplinary team experience preferred.
Knowledge of the process of death, dying and bereavement.
Knowledge and basic understanding of psychosocial and legal operatives in terminally ill patients and their families.
Knowledge and basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary healthcare team.
Current Kentucky driver’s license and reliable automobile.
Ability to exercise judgment and discretion with confidential matters.
Must make home visits to patients/families in various settings; mobile homes, apartments, nursing facilities, condominiums without elevators.
Must drive extended periods without ill effects.
Flexible, strong written and oral communication skills, and ability to give and receive constructive feedback.
Compliance with Bluegrass Care Navigators Employee Health, infectious disease, and vaccination policies.
Licensed driver with automobile in good working order, insured per state and organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
Salary $26.89-$42.96 per hour
