Overview
Ambulatory Social Worker - Care Transformation - FT - Days at Stormont Vail Health. Responsibilities
Participates in patient admission process (Inpatient, Outpatient, PHP, IOP) and assists as needed with signing of voluntary papers; serves as hospital representative and patient advocate. Coordinates and facilitates services for patients and their families throughout the Stormont Vail Health system. Performs social work assessments, considering bio-psychosocial, cultural, developmental and spiritual aspects of illness and hospitalization. Develops appropriate case management and/or treatment plans based on assessment and multidisciplinary team recommendations. Addresses patient/family understanding and adjustment to illness; establishes supportive/therapeutic relationships and communicates status and progress clearly; adjusts communication style as needed. Initiates discharge planning on admission; coordinates with internal and external multidisciplinary teams; develops long-term planning to reduce recidivism and facilitates referrals as indicated. Develops and facilitates groups for patients and/or significant others as needed. Maintains productivity, prioritizes patient care activities, and contributes to the development of new services in response to patient care demands. Maintains confidentiality and adheres to professional ethics and center standards. Participates in patient care conferences and provides patient education regarding diagnosis and treatment. Provides crisis and/or brief therapy for individuals, families, and groups. Assess patient and family functioning at discharge or referral time. Qualifications
Experience in customer service is preferred. Clinical and/or medical social work experience preferred. Experience in group work, case work, and community methodologies preferred. Licenses and Certifications: Licensed Bachelor Social Worker (BSW) - KSBSRB or Licensed Master Social Worker (MSW) - KSBSRB; temporary licensure may be accepted with full licensure achieved within 6 months of hire.
