No Weekends or On-call Required.
Location: 6526 2nd Ave, Detroit, MI 48202.
Schedule: Monday - Friday Day Shift. Flexible start and end time. 8 hour shifts.
$10,000 sign on bonus.
Who We Are Henry Ford Health Dialysis facilities (Greenfield Health Systems) offer compassionate, multi-specialty team, modern technology, clinical expertise, and latest medical knowledge to provide renal patients the highest quality care available. We have been part of the community since 1983 and are close to celebrating almost 40 years of excellence dedicated to dialysis.
Department Description 2nd Avenue Dialysis is a 48-station unit operating three shifts, six days a week handling complex patients from ages 20-90 years old. Many require assistance with mobility devices. Operating hours for patients are 5:00 am – 9:00 pm and team members 4:15 am to 9:00 pm. Employee schedules are made through several shift designs. Patients are medically managed by 9 Nephrologist and 2 Full Time Advanced Practice Practitioners (APP). We have Registered Dietitians and Social Workers plus a Social Worker Tech that helps with concrete patient needs such as transportation or dietary needs. Unit staffing includes a Nurse Manager, 2 Full Time Nursing Supervisors, many Registered Nurses, and Dialysis Clinical Technicians. We have additional support staff of Biomed Techs and Dialysis Assistants overseen by a technical leader.
What We Are Looking For
Masters-prepared Social Workers to work with our renal patients and families to assist with adjustment to what is likely a life-changing event.
Provide emotional and social adjustment counseling, assistance with removing barriers to treatment, education on kidney disease and treatment goals, and bridging barriers to referral services and community resources.
Conduct initial psychosocial interviews with patients and families to assess baseline level of functioning, comprehension of illness and its ramifications, and impact of individual/family history upon current and future psychosocial functioning.
Provide ego-supportive counseling to patients and families to maximize ongoing psychosocial functioning.
Provide crisis intervention to patients and families as needed.
Function as a patient advocate and as a liaison between the patient and multiple care delivery systems.
Participate in comprehensive inter-disciplinary team patient care meetings with appropriate documentation on IDT assessment and plan of care.
Record psychosocial assessments, update assessments and progress notes in accordance with established recording guidelines.
Education/Experience Required
Masters of Social Work Degree required.
LMSW (will consider LLMSW).
Experience in a healthcare setting preferred.
Demonstrates understanding of the impact of Chronic Kidney Disease (ESKD) on activities of daily living, quality of life and mental health needs.
Familiar with both clinical and case management techniques.
