Responsible for performing psychosocial screenings and assessments and implementing the psychosocial plan of care for patients to ensure the patient has sufficient resources and support services. Responsibilities

Performs psychosocial screenings and/or assessments to ensure patient needs are identified and appropriate interventions are integrated into the plan of care. Assesses patient and family psychosocial risk factors, prior functioning, support systems, coping ability, and patient/family priorities/needs. Collaborates with the patient, family and multidisciplinary team to address psychosocial needs. Uses medical records, patient/family interviews, and consultations with the care team to complete screenings/assessments/plan of care. Counsels patients and families regarding emotional, social, and financial consequences of illness and/or disability and other psychosocial issues identified. Provides crisis intervention and counseling; promotes patient and family empowerment and independence. Participates in discharge planning activities. Implements psychosocial portions of the plan to facilitate timely discharge and optimal utilization of resources. Plans for post-hospital placements and coordinates resources for safe discharge. Communicates with the interdisciplinary team regarding discharge planning status. Validates discharge criteria/plans and notifies the team of new resources or changes. Provides intervention in cases involving abuse/neglect, domestic violence, guardianship, foster care, adoption, mental health placement, and sexual assault as applicable; serves as liaison with CPS/APS as needed. Documents screening, assessment, interventions and outcomes to ensure effective communication per documentation procedures. Manages referrals and communications from various sources to ensure optimal customer service; utilizes Patient Support Center as appropriate. Maintains knowledge of applicable rules, regulations, policies, procedures, and guidelines (including The Joint Commission) and ensures discharge plans comply. Maintains professional relationships with management, vendors, hospitals, staff, and community agencies to ensure effective communication and timely discharge to appropriate settings. Reports assigned data (services provided, statistics, and related metrics) as needed with timeliness and accuracy. Identifies opportunities to improve work processes and customer satisfaction; makes recommendations and implements results in support of department goals. Stays current with developments in Social Work by attending seminars/workshops, reading, participating in professional organizations, and maintaining licensure; integrates knowledge into daily performance. Serves on committees and participates in peer reviews and meetings to promote quality and patient satisfaction. Qualifications

Master’s degree in social work. Experience:

None. Certification/Registration/Licensure:

Must be licensed by the Texas State Board of Social Work. For Behavioral Health/Psych Services Only (Cost Centers 60630 and 89200), must have a current Heartsaver CPR card within 60 days of hire/placement; current staff must obtain within 180 days from 4/19/2023. Acceptable providers include American Heart Association, American Red Cross, Military Training Network. Must have SAMA certification (level 1-4) before first clinical orientation shift; must obtain within 30 days of hire/placement. Current staff must obtain within 180 days from 4/19/2023. Skills/Abilities:

Ability to communicate effectively with diverse populations and teams; knowledge of community resources; excellent interpersonal and counseling skills; practice of patient-centered behaviors. Additional Information
