JusticeWorks is seeking a Family Services Case Manager / Plan of Safe Care Coordinator. In this role, you will be responsible for providing comprehensive case management services to children and families. Working with a team of professionals, you will assess the needs of each family in the diversion program, develop individualized goals, and coordinate services to assist families in navigating the social service system, provide crisis intervention, and help to promote positive change through advocacy.

You will also be responsible for coordinating meetings that focus on developing plans for families and children to ensure they receive the care and support they need to help ensure safe and healthy home environments. You will also work closely with the team to provide necessary resources and support for families in need.

RESPONSIBILITIES

Provide case management services

Complete assessments

Make referrals to appropriate agencies

Conduct outreach to obtain referrals from Children and Youth and Juvenile Probation and other sources

Conduct outreach and education to all medical providers and other social service agencies

Follow POSC procedures & ensure the program remains consistent with the model’s values, beliefs & philosophy

QUALIFICATIONS

Bachelor’s Degree required

Valid driver’s license, auto insurance and access to a vehicle

Pass current, applicable clearances

Pass pre-employment, post-offer drug screening

PAY RANGE $42,000.00/hr - $45,000.00/hr
