Job Overview
The Associate of Case Management is responsible for providing patient care management services for identified behavioral health patients involved in Henry Ford Allegiance Health Behavioral Health program. The role includes engaging patients in a collaborative process of identifying, planning, accessing, advocating, coordinating, monitoring, and evaluating resources, supports, and services to optimize functioning and well‑being. The Associate coordinates services within and across HFAH Behavioral Health programs, provides and coordinates referrals to community services, and facilitates transitions of care to maximize effectiveness and positive patient experience. The Associate functions as a core member of a collaborative care team involving the patient’s primary provider, consulting psychiatrist, and other health providers involved in inpatient and outpatient care.
Position Details
Full‑time, 40 hour week.
Education and Experience
Bachelor’s degree required. Bachelor of Social Work license in Michigan preferred. Minimum of two years working with behavioral health patients in ambulatory or community setting. Experience with patients with co‑occurring mental health, substance use disorder, and physical health concerns. Discharge planning and behavioral health case management experience required. Knowledge of utilization management, managed care, and payer issues preferred. Must be capable of standing, walking, stooping, bending, and lifting up to 25 pounds. Exceptional computer and analytical skills, ability to establish priorities, coordinate activities among multiple requests.
