ACT Rehabilitation Specialist - South Mountain
Terros Health is hiring an ACT Rehabilitation Specialist for our South Mountain clinic in Phoenix, AZ. Pay range $18.38 - $25.00 (DOE); $3/hour ACT Differential in addition to base pay. Full-Time, Employed. Location: 36th Street and Baseline in Phoenix, AZ, with approximately 20% in an office setting and 80% in the field. Bachelor’s Degree In Behavioral Health Preferred. Bilingual Differential Available. Must have very flexible schedule as this team provides treatment 7 days/week, including weekends and holidays. On-call and mileage reimbursement. Responsibilities
Provides direct treatment, rehabilitative and supportive services in a community setting to clients served on the ACT team. Works as a team member of a clinical team to assess member needs and wants, participates in the development of individual service plans, acts as a client advocate and liaison with families and community agencies. Serves as the team’s specialist for rehabilitation services by collaborating with other team members in the assessing for and delivery of rehabilitation skills training as specified in the individual service plan. Provides on-call crisis intervention services, as well as weekend and holiday coverage. Benefits
