Responsibilities
Oversee Care Management Services at a managerial level in your assigned EMA/TGA.
Utilize your understanding of Ryan White Services, contract deliverables, Standards of Care, and program/patient goals.
Collaborate with the AHF Healthcare Team, including Providers, the Practice Manager, the Associate Director, and your contract manager to ensure operational processes and deliverables align with goals and provide access to care for all patients.
Provide guidance and operational assistance to the team and monitor services entered into the Care Management Information System.
Ensure programs under oversight are audit-ready and that the team has the training and tools to implement operational goals.
Conduct 1:1 meetings with team members to review performance and resolve challenges, supporting patients in achieving healthcare goals.
Summarize program operations for the Associate Director of Care Management and address contract expectations to meet deliverables and improve processes.
Bring the principles of the Ryan White Care Act to the team for ongoing discussion and understanding; review deliverables to ensure targets are met and contribute to HAB measures and relevant regulations.
Advocate for the organization’s initiatives and align work with AHF Core Values and Mission in a dynamic environment.
Maintain an audit-ready Care Management program and ensure written policies and procedures comply with current EMA/TGA HIV Case Management Standards.
Collaborate with staff and leadership to execute the Model of Care and meet standards to enhance member care and outcomes.
May be assigned to other HCCs as needed.
Qualifications
Current and good standing Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) in the jurisdiction where services are rendered.
A minimum of 3 years of Case Management experience in HIV/AIDS is preferred.
Extensive knowledge of Ryan White Care Act as it relates to MCM contracts and overall Ryan White Care Act.
5+ years of management experience preferred.
Ability to travel in your region up to 50% of the time.
