We are seeking a qualified Social Worker to join our new Off‑Campus Emergency Department (OCED) in Lynnwood. The Social Worker will partner with the treatment team to ensure continuity and coordination of care for patients, families, and legal representatives, addressing social, environmental, psychological, financial, and cultural factors related to illness and hospitalization.
Pay:
$41.11 – $66.60 per hour
Shift:
6–7 hours per day, 20 hours per week (part‑time)
Responsibilities
Perform risk screening and provide services and consultation to patients and families regarding social, environmental, psychological, financial, and cultural factors related to illness and hospitalization.
Offer discharge/transition management support.
Assess for mental health and substance use crises, provide grief and loss support, and coordinate community resources.
Collaborate with Care Managers to manage appropriate utilization of resources and length of stay.
Maintain documentation and participate in ongoing quality improvement processes.
Qualifications
Master’s degree in social work, mental health counseling, or related field.
Valid license in Washington State (Licensed Independent Clinical Social Worker, Licensed Advanced Social Worker, Licensed Mental Health Counselor, or Licensed Marriage and Family Therapist).
Minimum of two years’ relevant experience.
Assessment and care‑planning skills; broad knowledge of community resources, medical terminology, hospital systems, insurance guidelines, financial issues, and risk management.
Effective verbal and written communication skills; computer proficiency; preference for experience with hospital documentation and quality improvement.
