UUP (State University Professional Services Unit) Job Summary:
In this position, the Social Worker is designated to inpatient medical/surgical units at the downtown campus. The Social Worker provides comprehensive psycho-social assessment to identify and address social drivers of health while working collaboratively with the multidisciplinary team to support the needs of patients and families. The candidate shall gain knowledge of social determinants, population health, and drivers of utilization. Duties will include rounding with the multidisciplinary team on assigned units, consultation including assessment, brief counseling, and intervention. Candidate shall learn to initiate and implement advance directives and will assist with discharge planning recommendations and referrals to meet the psycho-social needs of patients and families. Candidate must possess strong verbal and written communication skills and have knowledge of available community resources. The Social Worker will participate in departmental meetings/in-services, education, and quality improvement activities. The Social Worker performs other relevant duties as assigned, including back-up coverage for other Social Workers and occasional holiday coverage. Minimum Qualifications:
M.S.W from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master\'s experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit. Work Days:
Monday-Fridays, days with occasional on-call/holiday coverage on rotation based on operational needs Message to Applicants:
