Overview
Social Worker II/III - LMSW (Family Medicine- Bronx) role at URBAN HEALTH PLAN, INC. The Social Worker provides patients with supportive counseling, crisis intervention, psychoeducation, advocacy, and connections to support services. Bilingual (English/Spanish) candidates are sought for full-time opportunities within Bronx-Based Health Centers (Boricua and Tres Puentes Community Health Centers). This is a hybrid role requiring in-person work at Boricua 3 days/week, 1 day at Tres Puentes, and 1 day remote from home. Responsibilities
Conduct psychosocial assessments, provide supportive counseling, complex case management, and crisis intervention. Collaborate with the interdisciplinary team to assess and address patients' psychosocial barriers to care. Refer and connect patients with services including behavioral health treatment and community-based organizations. Document all in-person and telephone encounters with patients in a timely manner. Maintain patient confidentiality and HIPAA regulations per UHP policy. Qualifications
Master's degree in social work, required. NYS LMSW Certification, required. Experience and/or training with crisis intervention. Excellent communication skills. Dedication to supporting patients in our communities. Core computer skills including ability to maintain electronic health records. Bilingual Spanish/English is required. Starting Compensation
Minimum $67,600 per year; Maximum $75,000 per year Work Environment, Location, and Schedule
Hybrid role: 1 day remote, Bronx, NY; Monday to Friday 8:00am-5:00pm. Employee Benefits
