Position Overview This is a full performance level Professional Social Worker who provides Social Work Services to chronically ill and elderly residents. The Social Worker conducts home visits to identify services for individuals who are at risk of Nursing Home Placement. Population served includes infants, youth, adults and seniors.

Responsibilities

Provides direct social work services by obtaining pertinent information from clients and family members, to assess socio-economic, emotional and health problems.

Document client’s assessment, treatment/intervention and progress in agency medical records in accordance with Health Department, State and Federal guidelines.

Establish a Person-Centered Plan of Care, recommend services and provide resources to clients and family members.

Confers with health professionals, Supports Planners, family members and other designated individuals on the care needs of clients.

Complete weekly/monthly reports.

Qualifications

Graduation from an accredited college or university or equivalent program as determined by the Maryland State Board of Social Work Examiners with a master's degree in social work.

Must become licensed as a Licensed Certified Social Worker (LCSW) certified by the Maryland State Board of Social Worker Examiners within one year.

Possession of a valid license to operate a motor vehicle when required for the performance of job-related duties at time of appointment.

Any equivalent combination of relevant training, education and experience may also be considered.

Conditions of Employment

Meet all training and performance standards and demonstrate proficiency as required by the agency.

Wear and use agency protective apparel and equipment in the performance of their assigned duties, if applicable.

Successfully pass preemployment checks which may include reference checks, background investigations, and drug screenings, where applicable.

Be willing and able to serve as an essential employee. Essential employees are expected to report during standard or non-standard hours as operations necessitate, or during emergencies. Essential employees are expected to report or remain at work when other County employees are granted Administrative Leave.
