Social Worker (MSW, LSW)
Location: West Hospital, Avon, Indiana Shift: Full-Time Days (may include on-call and holiday rotation)

Indiana University Health West Hospital in Avon, Indiana, is seeking a compassionate Social Worker (MSW, LSW) to provide comprehensive psychosocial support, crisis intervention, discharge planning, and case management for children and families. Position Overview The Social Worker meets face-to-face with patients and families, collaborating with the healthcare team to ensure safe, timely, and resourceful care. Responsibilities include:
Interpreting and communicating complex patient and family needs
Modeling caring practices for other caregivers
Assessing psychosocial risk factors, prior functioning, and support systems
Counseling on emotional, social, and financial impacts of illness or disability
Mobilizing community and family resources to meet patient needs
Conducting mental health assessments upon physician referral
Facilitating complex discharge planning and crisis interventions
Serving as the point person for cases involving child abuse/neglect, elder/abuse, domestic violence, and institutional abuse
Acting as liaison with child and adult protective services (APS/DCS) and community agencies
Providing specialized mental health services, including crisis and addiction assessments
Supporting acute crisis situations with de-escalation techniques to prevent violence
Leading goals of care conversations with patients, families, and care teams
Supporting end-of-life, guardianship, foster care, and adoption processes
Qualifications & Requirements Education:
Master’s Degree in Social Work (MSW) required.
Licensure:
Permanent LSW licensure required.
Certifications:
Crisis Prevention Intervention (CPI) or equivalent de-escalation training within 6 months of hire.
Experience:
3+ years of relevant experience in mental health, child and family services, or hospital case management.
Knowledge & Skills:
Evidence-based psychosocial assessments and crisis intervention
Knowledge of disease processes, human behavior, and social needs across the lifespan
Complex discharge planning skills
Legal mandates for protective services (child and adult)
Ability to adapt services for diverse populations
Strong relationship-building with hospital staff, law enforcement, community agencies, and families
Computer literacy and proficiency in electronic medical records
May be scheduled for on-call and holiday shifts
